I am an in-network participating provider with Blue Cross Blue Shield. I am an out-of-network provider for all other insurance plans. This means that you pay me and I will provide you with paperwork that you submit to your insurance company for reimbursement.

You have the right to receive a Good Faith Estimate explaining how much your medical care will cost.
Your therapy time is reserved for you. Cancelled or missed appointments will be billed to you unless you provide notice 24 hours in advance.
Information about your treatment will be exchanged only when you have given written consent. There are some instances when I might be legally required to disclose information without your permission, including when there is imminent/serious danger to you or others, when child abuse is suspected or reported, or when compelled by a court in the administration of justice.
Phone messages will be returned as soon as possible. Please be advised that this is an outpatient psychotherapy practice and not critical care. In case of an emergency, please call 911 or seek help at a local hospital emergency room.
